Description of Services:

Offers home support, nursing and respite services provided in client's home or in hospitals, assisted living homes, long-term care facilities, schools and offices * all clients received an initial assessment completed by a Registered Nurse * free in-home care assessment

Home support includes:

personal care
homemaking
companion aide
palliative care
Alzheimer's and dementia care

Nursing includes:

wound care
intravenous care
palliative care
acute care post hospitalization
chronic disease management
pediatric services
mental health
foot care services (in-home foot care available)
school health

Respite care:

provided for shorter or longer periods of time
right level of staff to meet the care and support needs
