Description of Services:
In collaboration with Adult Day Services team members and other health care professionals, support and care provided to caregivers to help them balance needs and responsibilities:
caregiver support, education, and counselling through the health care spectrum
time limited, solution focused therapeutic counselling onsite, over the phone, or in a support group for both client and caregivers
education on resources and services available to enhance client autonomy
advance care planning discussions
referrals to other health professionals as appropriate
