Description of Services:

Support for people 18 years and older living with pre-diabetes or Type 1 or Type 2 diabetes * one-on-one or group sessions * education and management on monitoring blood sugar levels, medications, healthy eating, exercise and other lifestyle choices * registered nurse and dietitian on staff
