Description of Services:
Community support services to promote successful discharges from either in-patient hospital stays or emergency departments * provides people services to regain strength and independence.

Services may include:
  • frozen meal delivery
  • transportation home from hospital
  • transportation for follow up medical appointments
  • homemaking services
  • home safety assessment
  • wellness check ins
  • communication with other support services
