Description of Services:
Program for Adult Day Services clients living with dementia * helps seniors age at home safely * the program gives clients access to a specialized primary care led team that works together to solve complex client and caregiver needs. The interdisciplinary team includes primary care, a Neurobehavioural Nurse Practitioner, a Registered Nurse, a Registered Social Worker, and the entire Adult Day Services team trained in emotion-based care

Services include:

  • dementia-focused primary care
  • specialized neurobehavioral and geriatric services
  • pharmacy consultation support for high-risk medications
  • lab work and diagnostics coordination
  • access to social work services and social care supports for clients and caregivers
  • outreach to other care partners for care planning and supports that prioritize aging at home

Additional requirements for the Thrive Program:

  • 65 years or older
  • willing to change from current primary care provider to physicians at the Integrated Care Centre within the Seniors Health and Wellness Village at Peel Manor in Brampton
  • able to attend in-person adult day services for minimum of 2 months, before switching primary care provider
  • overall goal of staying in the community vs. transitioning to long term care
  • insured under the Ontario Health Insurance Plan
  • supported by an engaged caregiver
  • cognitive impairment (CPS score of 2 or more) or diagnosis of dementia
MAPLe score of 3+ with high risk of:

  • emergency admission or readmission to hospital
  • alternate level of care status if admitted to acute care
  • crisis placement for long term care
