System navigators providing information and referral to match people with services that will address their health and social needs

Referrals include:
  • services to address speech and language issues, sensory concerns, social skills
  • family support services for sibling support, parent groups and respite
  • general issues for sleep problems, haircuts, eating and dentist visits
