Description of Services:
Intensive community-based treatment and support program for people living with serious mental illness, particularly schizophrenia, bipolar disorder and schizoaffective disorder, who require up to daily contact * for residents of Guelph and Wellington County

Services include:
  • medication monitoring
  • crisis assessment and intervention
  • symptom assessment and management
  • supportive counselling
  • life skills teaching
  • housing assistance
  • vocational support
  • advocacy
