Description of Services:
Outpatient clinic providing care to adults who have a new diagnosis of heart failure and/or recurrent symptomatic heart failure * services include:
- general education about heart failure
- medication adjustments
- investigations
- assessments by nurse practitioner, pharmacist, social worker, or dietitian
- self-monitoring strategies to reduce risk of symptoms reoccurring
