Program provides assessment and recommendations for pain management plans to individuals with chronic pain. Recommendations are sent to the primary care provider and shared with patients. Counselling and self-management classes are available. 
Visits are either in person or virtual. 

Referrals are accepted from primary care providers and community healthcare agencies.
