Eligibility / Target Population
One-On-One Services: Must live in the city of Burlington or have a family physicians located in the city * adult 20-64 years * not eligible for OHIP * no extended health benefits * no active claim or litigation for WSIB, motor vehicle or personal injury
Group Programs: Must live in the city of Burlington or have a family physicians located in the city * must be 20 years of age or older
Eligibility is subject to an assessment by a BFHT Registered Physiotherapist

Description of Services:
One-on-one and group-based physiotherapy for adults who have limited or no access to physiotherapy services * progression and maintenance levels of rehabilitative care for patients working to restore or maximize functionality, or prevent functional decline or injury, or to maintain functionality * individual service treatments include:
Pulmonary Rehab - An 8-week education and supervised exercise program for individuals living with COPD or other chronic lung diseases
GLA:D Canada - A 7-week, research-based education and exercise program for people with hip and/or knee osteoarthritis
EFEA (Exercise For Everyday Activities) - A 7-week, education and function-focused exercise program for people with balance and mobility issues
LIFT - A 7-week, education and exercise program for people with shoulder pain * exercise is focused on regaining movement and functional abilities in the injured shoulder
- osteoarthritis
- sports and athletic injuries
- rotator cuff and shoulder injuries
- chronic pain and fibromyalgia
- COPD and other breathing difficulties
- balance issues and fall prevention
- vestibular conditions, including vertigo
