10-week program for people who have Chronic Obstructive Pulmonary Disease (COPD), including those recently hospitalized due to COPD * virtual services available

peer support
caregiver support
group and individual COPD education
personalized exercise programs
personal counselling

Progression level of rehabilitative care for patients working to restore or maximize functionality
