Adults 18 years and older with significant physical/functional impairments * must have physical tolerance to participate in two or more hours daily of therapy * must require two or more services (not including nursing) * patient's anticipated discharge destination cannot be a long-term care home

Description of Services:

Inpatient program providing low intensity, longer stay treatment and rehabilitation therapy for people with multiple medical and/or functionally complex conditions * services provided by a physician, nurse practitioner, social worker, physiotherapist, occupational therapist, rehabilitation assistants, speech language pathologist, communicative disorders assistant, dietitian, pharmacist, nurses, registered practical nurses, respiratory therapist, and clinical nurse specialist/wound care specialist
