Description of Services:
Provides timely, short-term, community-based mental health and addiction support to individuals experiencing heightened vulnerability. The program helps individuals stabilize, work on goals and connect to additional appropriate supports
In-STED is designed to reduce repeat and non-essential visits to emergency departments by addressing needs earlier and more holistically. The team works collaboratively with individuals, families, and community partners to provide short-term case management supports, service navigation, and warm linkages to ongoing community-based services
Referrals to the program are received from Trillium Health Partners, supporting seamless transitions from hospital to community care and promoting continuity, dignity, and recovery-focused outcomes
