Helps with daily activities and supportive services to maintain independence in the community at designated buildings * a hub model providing support to the community as well as the designated buildings

• assistance with personal care
• meal preparation
• medication reminders
• housekeeping and laundry

Frequent visits, both scheduled and unscheduled, are provided throughout the day
