Apply through the City of London Housing Access Centre for rent-geared-to-income * apply directly for market rent * assessment at Trinity for to ensure needs can be met
Eligibility / Target Population
Older adults and adults with disabilities 65 years and older, who need physical, social, or emotional supports * 60 years and older for wheelchair units * clients must be able to live independently with basic assistance

Description of Services:
Independent living with basic supports

Package includes:
  • daytime access to personal support worker for health service coordination
  • light maintenance and annual cleaning of apartment
  • daily monitoring of apartment via motion sensor
  • social activities and programs
  • personal care and additional housekeeping can be purchased
  • personal care may be available through Ontario Health atHome for those who are eligible
