Description of Services:
Programs and services for people with mild cognitive impairment, Alzheimer's disease and other dementias and their care partners
- Education Programs and Services Support programs and services for people living with Alzheimer's disease and other dementias, Mild Cognitive Impairment (MCI), and their care partners.
- Finding Your Way ID Kit Finding Your Way Ontario provides public education and an ID kit to assist in a search for missing persons. With consent, ID kits are completed by registered clients and faxed to local police for entry into their database, so all relevant information about the person living with dementia is already stored and ready to be used in the event of an emergency. Contact your Social Worker or contact@alzswp for more information.
- In-home Recreation Program Our In-Home Recreation program provides individualized, one-to-one recreation support for people living with dementia in their own home. Delivered by a Therapeutic Recreation team member or trained volunteer, visits focus on meaningful engagement, enjoyment, and well-being while offering care partners valuable respite. Personalized recommendations and resources are provided to support continued engagement at home

At AlzSWP, both the individual and the family are provided with support upon diagnosis and throughout the continuum of the disease.
