Application
Patients may be referred while still in hospital * accepts referrals from specialist or physician * assessment required prior to admission
Eligibility / Target Population
Adults 19 years and older who have had heart surgery or a diagnosis of cardiac disease within the last two years

Description of Services:
Supervised exercise and education program for adults who need cardiac rehabilitation * depending upon risk factors, comfort with exercise and preference, patients will be offered an exercise program at the YMCA or a personalized home exercise program with weekly contact from the rehabilitation team
