Eligibility / Target Population
Older adults, adults with disabilities, people with Alzheimer's Disease and related dementias who need physical, social, or emotional supports * care coordinators assess the needs of applicants
Fees
$13 per day * additional $13 fee for transportation (one way), $15 round trip

Description of Services:
Adult day program * social, recreational, and therapeutic activities for adults 18 and older, in a group setting for part of the day *  helps to support family caregivers who provide day-to-day care
  • meals
  • individualized social and recreational plans for each client
  • caregiver support
  • blood pressure, weight and medical monitoring (by a registered nurse or registered practical nurse)
  • foot care (for a fee)
