Description of Services:
Supports for older adults, and adults with disabilities * helps people remain independent in their own homes for as long as possible
- Assisted Living On-site personal support services for eligible individuals * helps with daily activities and supportive services to maintain independence * a hub model providing service to the community as well as designated buildings * sites include Maplewood and Birchwood Apts in Zurich
- Dining for Seniors Hot meal and conversation for seniors
- Exercise and Wellness Focuses on older adults maintaining and improving their overall health
- Friendly Visiting and Telephone Reassurance A support program that matches a volunteer with a senior who is isolated, lonely or could use a friend * the client and volunteer meet on a one-to-one basis and visit in the client’s home * these regular visits can include a telephone conversation, or enjoying activities outside the home
- Home At Last Program Assists older adults with transitioning from hospital to home
- Home Help Assistance with daily chores, such as laundry, light housekeeping, heavy cleaning, yard work, errands, shopping, and light meal preparation * Home Help workers' visits can be done working with the client's schedule and preferences
- Lifeline Emergency Response Medical alert system * allows older adults and people with disabilities to call for assistance using a help button * 24-hour emergency response
- Meals on Wheels Hot meals delivered Mon-Fri between 11:30am-1pm * special diet types available
- Overnight Respite Overnight relief for caregivers of older adults, frail elderly and those with Alzheimer's disease and related dementias * accommodations are offered at the Wingham Adult Day Centre * trained staff provide assistance with activities of daily living, personal care and structured activities in a safe environment
- Personal Support and Homemaking Services In-home supports for the activities of daily living * personal care support worker helps with personal care
- South Huron Adult Day Program Adult day program * social, recreational, and therapeutic activities for older adults, adults with disabilities, and people with Alzheimer's disease and related dementia in a group setting for part of the day * helps to support family caregivers who provide day-to-day care
- Transportation - EasyRide Non-emergency transportation to local and medical appointments and regular activities such as shopping, errands, or social activities * volunteer drivers * wheelchair accessible
